This year will mark the fourth year of our annual presentation evening to mark the incredible achievements of the players involved at this very credible level.

The evening will bring together the players from the Centre of Excellence and offer various awards and prizes as well as food and entertainment.

Overleaf is the nomination form for the evening. Each group will be able to nominate the Players player award; there will also be a manager's player award and a special achievement award given across the age groups. Players will also be awarded a trophy/medal to acknowledge their very special achievement of making the Centre of Excellence 06/07 Season.

The evening will be held on Thursday 26th June , at the Blackburn Rovers - Blues Café/Bar. The evening will commence at 7:15pm so we ask you arrive between 6:30 and 7:00pm. There will be a buffet served at the end of the presentation and a fully licensed bar is available to the parents.

Blackburn Rovers FC will of course cover the costs of the evening for the players, although we are insisting that parents/carers/friends & family purchase the presentation tickets. These can be purchased on the evening of the event or pre-ordered by returning this slip. The costs of the tickets are £5.50 per person.

*Please note that parents must indicate by completing the form as to how many tickets they will be purchasing if doing so on the evening of the presentation. All attendees will be subject to the admission charge of the above amount.

The closing date for posted applications is 31st May.
